This referral form is to access our scaffolding service. If your referral is accepted, we will offer you an invitation to one of our Forensic Liaison Clinics (FLC). An FLC is a way for you to discuss risk behaviours of a patient you are working with. Within the FLC, we will have a brief discussion about the patient. We may ask for updates since the referral was made. We will then provide advice and guidance for you to help devise a plan moving forward, based upon our experience with working with patients who pose a risk to others. This may also lead to further scaffolding and support from the Forensic Community Service. For FLCs, we require the Care Co-ordinator/Referrer to be present; along with any other key professionals involved in the patients care.
The Forensic Community Service ordinarily only Care Co-ordinates those patients coming directly out of the secure in-patient pathway. If you would like to discuss this further, please contact:
