The North Tyneside Primary Care Physical Health Service supports adults with mental health needs to access physical health checks. The team works with GP surgeries and can offer appointments at home, in a clinic, or another agreed place. These checks can help spot health concerns early and support people to stay well.

This leaflet provides information about our service and how we can support you. We will work with you to help you get the right physical health checks at the right time.

We are a team that helps you get physical health checks through your GP surgery.

The North Tyneside Primary Care Physical Health Service supports people who may find it hard to go to their GP surgery, or who cannot go to appointments there.

The team can meet you at home, at a clinic, or at another place we agree with you.

We work with your GP (family doctor) and practice nurse to help with your physical health and how you feel.

Some medicines for mental health can affect your physical health. These checks can help find any problems early.

We help make sure you have the right physical health checks at the right time, so you can stay as healthy as possible.

The service is for adults in North Tyneside who have mental health needs and may need support to access physical health checks.

You cannot ask to use this service yourself. Your GP, nurse, social worker or support worker can ask the service to see you.

The team will look at the information they are sent. This helps us decide if this service is right for you. If we can help, we will contact you to book an appointment.

Your first appointment/assessment

Your first appointment is where we complete your physical health checks and talk with you about your health and wellbeing.

Checks may include your blood pressure, weight, blood tests and a discussion about things like smoking, alcohol, food and exercise.

Ongoing support

We usually offer one appointment to complete your physical health checks. Your GP may ask us to see you again if you need more checks.

Reviews and discharge

When you no longer need support from our team, your care will continue with your GP or another service if needed. We will make sure you know where to get support in the future.

The team is made up of a manager, nurses and clinical support assistants. They work together to support your physical health and how you feel.

  • Nurses can give advice and support about your physical and mental health.
  • Clinical support assistants can help with physical health checks. This may include taking blood tests.
  • The team may speak with your GP, doctor or practice nurse to help plan your care.

You may not need to see everyone in the team. Only the people who are right for your needs will be involved in your care.

Your care will be planned with you. The team will listen to what matters to you and talk with you about the checks or support you may need.

Some medicines for mental health can affect your physical health. Physical health checks can help find any concerns early.

We understand that family members, carers, or other people who support you may be important to your recovery.

With your permission, the team can involve your family, carers, or supporters in your care. This may include inviting them to appointments or sharing information that helps them support you.

We will always respect your choices and your right to privacy. We will only share information with other people if you agree, unless there is a serious safety concern.

Appointments

Appointments are usually arranged by the Team. This may be by letter, phone call, text message, or other agreed method.

Your appointment may be at a clinic, in your home, by phone, or by video call. We will agree this with you.

If you cannot attend an appointment

If you cannot attend, please contact the team as soon as you can. We can then offer the appointment to someone else and find another time for you.

If you miss appointments and do not tell us, we may need to review how we support you.

Contacting the Team

The team will tell you the best way to contact them and when they are open. This may include phone or email.

If you need help when the team is closed, contact your GP, NHS 111, or your local crisis service. In an emergency, call 999.

You can call NHS 111, option 2, for urgent mental health advice at any time. This service is open 24 hours a day, 7 days a week.

You will speak to a mental health worker. They will listen and help you find the right support.

If you are Deaf or have communication difficulties, you can contact us in a crisis 24/7 by:

Call 999 or go to Accident and Emergency (A&E) if:

  • you, or someone else have harmed themselves or is at immediate risk
  • there is a medical emergency and someone is seriously ill or injured

If you are not in your local area and need urgent help from a local crisis team, you can call the hospital switchboard on 0191 246 6800.

Being involved in decisions

You have the right to be involved in decisions about your care and treatment. The team will listen to you, explain your options, and answer any questions you may have.

We will work with you to agree what support is right for you, based on your needs, wishes, and goals.

Consent and confidentiality

Your information is kept private and confidential. This means we do not share information about you without your permission.

We will ask for your consent before sharing information with family members, carers, or other services, unless there is a serious concern about safety.

You can ask questions at any time about how your information is used.

Interpreters, accessibility, and reasonable adjustments

We want to make sure you can access our service in a way that works best for you.

Interpreters can be provided if you need support with language or communication. Information can also be made available in other formats, such as large print or easy-read.

Please tell the team if you need any changes to help you access the service, such as extra time, communication support, or help getting to appointments.

All our main outpatient departments are easy to reach by public transport.
