Who are we?
We are the Forensic Learning Disability Community Transitions Team (CTT).
We work at Northgate Hospital.
We also see patients from North and South of the Tyne.
We are a team of different health workers. We work together to support you.
Our team includes:
- Assistant Practitioners
- Nurses
- Occupational Therapists
- Psychiatrists
- Psychologists
- Speech and Language Therapists
Staff can tell you what we all do.
What is transition?
Transition is when you move from hospital to a home in the community.
We support you during the move.
What do we do?
We will help you plan your move from hospital.
This is a discharge plan.
We will get to know you while you are still in hospital.
We will support you before, during, and after your move.
We will attend meetings about your care and future home.
We will work with you, your family, and other people involved in your care.
We will meet with you often.
We will check how things are going.
We want you to feel safe, happy and supported.
We will work with the hospital and your new care team.
This helps make sure you get the support you need.
If there are problems let us know.
We will work with you and your care team to help.
We will continue to support you after your move.
The amount of support will depend on your needs.
