How to refer

Service criteria:

- A diagnosis of a Traumatic Brain Injury (TBI) which may be new or historic
- 16 years of age plus
- Northumberland resident or registered with a GP in Northumberland
- Have a specific goal(s) that we can help you with

Referrals are accepted via GP, Healthcare Professionals, Carers or by Self referral.
