How can I get referred?

Referrals are reviewed weekly through a Single Point of Access (SPA) meeting, attended by Clinical Coordinators from multiple disciplines and chaired by CSPT’s Pathway Manager.

To ensure timely processing, please complete this referral form with as much detail as possible. Please note we do not accept self-referrals, however the referral form can be completed jointly by client and health care professional.

Referrals must be submitted by Tuesday at 12 noon to be considered at that week’s SPA meeting, which takes place every Thursday. We aim to provide written communication following each meeting.

What to expect:

  • Multidisciplinary Review: Referrals are assessed by a team of professionals who may recommend different or additional support approaches.
  • Joint Assessments: If another approach is suggested, the referred individual may need a joint assessment across multiple services. We advise informing them of this possibility in advance.

Next steps:

  • Accepted Referrals: Service users will be invited to opt in for an initial assessment to determine suitability for the recommended approach.
  • Assessment Outcome: As the referring professional, you will receive a copy of the outcome. If the individual does not meet the service criteria, CSPT will notify you accordingly.
