Providing a single point of contact for access to multi disciplinary health and social care teams who offer specialist assessment, treatment, and care for older people with mental health problems arising from functional or organic disorders.
The community treatment team also provides primary care assessment, intermediate care, planned home treatment, challenging behaviour, rapid response, liaison, younger person with dementia, memory management services, family therapy, psychology, physiotherapy, occupational therapy and medical support.
