Northumberland Community Learning Disability Team – CNTW135

The Northumberland Community Learning Disability team is a specialist multidisciplinary team, covering the county of Northumberland. There are bases in Hexham, Blyth, Alnwick and Ashington. The team is staffed by a range of specialist learning disability health professionals with training and experience of working with individuals and their families, carers and supporters, including community learning disability nurses, nursing associates, clinical support workers, psychological professions, physiotherapists, non-medical prescribers, psychiatrists, team managers and administrative staff.

The team aims to provide assessment, formulation and evidence-based interventions for adults with a learning disability and additional health needs, which require a specialist and/or integrated approach to care and treatment, which cannot be met through mainstream services.

The team helps people with:

  • Physical health
  • Mental health
  • Behaviour support and keeping people safe
  • Medication to help with mental health
