Learning Disability In-Reach and Transitions Team (LDITT) – CNTW309
The Learning Disability In-Reach and Transitions Team (LDITT) works across Northumberland and Tyne and Wear. We support individuals with a learning disability, or a learning disability and co-occurring autism.
The Team aims to enhance care within mainstream adult mental health inpatient services by providing specialist learning disability support.
This includes:
- Supporting people with learning disabilities, including those with co-occurring autism within inpatient mainstream wards
- Supporting a timely, successful hospital discharge
- Building staff skills and knowledge
- Improving experiences for individuals, families, and carers
We work closely with clinical teams across to provide specialist input, guidance and joint working to enhance outcomes and experiences for the people we support.
