Northumberland Older People’s Intensive Response Team (NOPIRT) – CNTW211

Northumberland Older People’s Intensive Response Team (NOPIRT) work within the older people’s pathway. The team aims to offer specialist support to the care and treatment of patients to reduce and prevent avoidable hospital admissions, avoid delays in discharge and deliver services to people who would otherwise face unnecessary prolonged hospital stay.

The team will be working with people whose health needs would previously require a 24 hour therapeutic hospital environment and can be accessed for all Northumberland patients who would require admission for organic assessment.

The team will focus on preparing patients for timely and effective discharge into care homes or back to the patient’s own home.
