Specialist Community Forensic Team (SCFT) – CNTW272

The Specialist Community Forensic Team (SCFT) covers the foot print of the Secure Provider Collaborative (including the two Trusts TEWV & CNTW), providing specialist support, intervention and scaffolding with patients within Secure bed based services and those in the community. They enable step up and wrap around theraputic interventions to reduce risk and prevent relapse delivered by a robust multi-disciplinary team (MDT)/Peer Supporters
