The Northumberland Community Learning Disability Team support people who have been referred to the service with a variety of needs:
- Care and support
- Physical health
- Physical health appointments
- Reasonable adjustments
- Mental health
- Behavioural support
- Emotional wellbeing
- Carers needs
- Relationship support
- Support with mental capacity Best Interest assessments
- Communication passports linking in with SLT
This list is not exhaustive. Further needs and support can be identified during nursing assessments completed following individual referrals. The team work collaboratively with adult social care Learning Disability Pathway colleagues to ensure a holistic approach.
