The Forensic Community Team consists of community psychiatric nurses, psychiatrists and a social worker. The Forensic Community Health Team is a specialist tertiary service commissioned by NHS England. Its purpose is to contribute to reducing the risks to others posed by men and women who have significant mental illness. This includes those who have been assessed as posing a significant risk of harm or those who are thought to do so and require further assessment of their risk. The team seeks to optimise the mental health of individuals and improve their quality of life. Additionally, the team seeks to educate other professionals in the assessment and management of the risk of harm to others.
